2. Magellan: A Fatal Miscalculation on an Oil Rig

Gordon Moffat, a 41-year-old "roughneck" working on the Magellan offshore oil rig, faced a tragic workplace accident. tasked with fixing a drill cable, Gordon was lowered into a tight space beneath the main deck via a winch system. Due to a communication failure and a mistake by the hoist operator, the winch was not reversed; instead of being lowered, Gordon was hoisted upward toward a narrow 10-inch opening known as a "mouse hole."

Despite the crew's desperate attempts to stop the hoist, the mechanical force pulled Gordon into the opening, effectively crushing him. The rig's operator was later found guilty of blatant delinquency regarding safety protocols, resulting in a £60,000 fine and a settlement with the victim's family.

3. Boilermakers: The Tragic Disappearance of Wade Steffi

In January 2007, 19-year-old Purdue University freshman Wade Steffi went missing after being denied entry to a dormitory. In an intoxicated state, he sought an alternative entrance to retrieve his jacket, leading him to an off-limits door at the basement level of Owen Hall. This door provided access to an electrical vault containing high-voltage transformers.

While navigating the pitch-black room, Wade accidentally wedged his finger into a two-inch hole containing an exposed electrical conductor, exposing his body to between 2,000 and 4,000 volts of electricity. He died instantly, remaining propped against the equipment for nearly two months until a maintenance worker discovered his body after hearing a strange "popping sound" caused by electrical arcing. Purdue University was eventually found negligent for failing to secure the maintenance access, leading to a settlement and the creation of a scholarship in Wade's name.
