  • Gender Bias: Because migraine disproportionately affects women, it has historically been "not taken very seriously" in clinical settings, which subsequently impacted funding priorities at institutions like the NIH.
  • Linguistic Stigmatization: The word "headache" is often used metaphorically to describe minor annoyances like "sitting in traffic" or "doing your taxes." This trivialization obscures the reality that for millions, these are severe "neurobiological disorders."
  • Scientific Stigma: Zeller interviewed numerous scientists who were warned during their training not to pursue headache research, as it was considered "not a very sexy thing to study" and lacked the perceived prestige of fields like Alzheimer’s or movement disorders.

The Path Forward: Hope in Neurobiology

Despite these challenges, Zeller remains optimistic about recent advancements. He highlights the development of CGRP (calcitonin gene-related peptide) medications, which hit the US market in 2018. This breakthrough followed 30 years of research into neurochemicals that are found in "abundant supply in the blood" during migraine attacks.

While these drugs are not a panacea—Zeller notes that in the world of drug development, helping half the patients experience half the number of headaches is considered a "home run"—they represent a significant leap forward. Researchers are now actively investigating other neurotransmitters and potential targets, moving away from the outdated view that headaches were merely "psychosomatic" or strictly "vascular" issues.

Ultimately, Zeller’s work serves as a call to recognize headache disorders as legitimate, complex neurological events.
