Initially, the hospital administration feared an act of sabotage or an "enemy conspiracy" related to the war effort. Governor Charles Sprague was deeply concerned that Axis powers might have poisoned the federal food supply. However, subsequent toxicology reports confirmed the substance was sodium fluoride, and experiments conducted by the hospital superintendent, Dr. John Evans, using rats, proved that the contamination occurred locally rather than through a tainted supply chain.

Investigations revealed that George Nosen had accidentally added five pounds of the poison to the eggs, believing it to be milk.
