It's Dr April and welcome back to all of you marvelous, magnificent mini medical students listening in.
Hey, if you need to refresh your memory about where we are, take a quick listen back to our earlier episodes.
Okay, let's get right to it.
Come on Dr. April, hurry!
I'm coming.
It is great to be back.
Sounds like they're all having a really great time.
Okay, I'm pretty sure I think I know who done it.
You are.
Me too.
Hand, foot, mouth, Hattie.
Hmm.
What do you think, many med students at home?
Interesting.
We need to settle this once and for all and quickly.
Kiara really needs our help.
So let's debate.
Dr. Waffle, Dr. Cespedes, tell us why you each think your conclusion is correct.
Lay out your clues and use your fact frames and what we've learned about your suspect.
I am taking the position that Fungi is guilty of causing Kiara's rash.
Reason 1.
Fungi is known to cause rashes according to Kiara's fact frames and physical exam.
She indeed has a rash on the front and back of both her hands.
Reason 2 fungi doesn't cause fevers like the other two suspects do and according to kiara's vital signs she doesn't have a fever.
Reason three fungi.
Already knows kiara she's had a ringworm before.
In my sources report that it's common to get it again.
Therefore, I conclude that the facts clearly support fungi as a culprit here.
Thank you.
Excellent use of the facts, Dr. Waffle.
Dr Cespedes, care to offer a rebuttal and tell us your position based on the evidence indeed.
Thank you, dr april and dr waffles.
I yield the floor.
Fungi couldn't have caused kiara's rash.
Reason one Chiara's rash is red, itchy and lumpy.
While fungi does in fact cause a rash, ringworm doesn't look like what we saw in Chiara's focused physical exam.
According to Miss Immunity, ringworm is a round rash with a raised border, redness and flaking of the skin.
Reason 2.
The timing of Kiara's rash doesn't fit with a ringworm like fungi.
Kiara's fact-framed story shows that she noticed it suddenly while completing her art project.
Ringworm has an incubation period of a couple days.
In other words, there's about two days between the time a person is exposed to ringworm and when the rash actually appears.
Reason three fungi isn't known to cause fevers, coughs or rhinorrhea like some of our other suspects.
And while Kiara doesn't have a fever, she definitely has rhinorrhea, a cough, and even a wheeze.
The whistling noises we heard on Kiara's lung exam.
Precisely.
Therefore, fungi couldn't have caused Kiara's rash.
Outstanding arguments and counter arguments, mini doctors.
Mini med students listening in, what do you think?
Which argument was the most convincing?
Do you believe fungi is our culprit?
Which of our mini doctors do you agree with and why?
Here's some thinking music to think about it.
Many medical students, you are thinking, and I love it.
But hold those thoughts and notes.
We have one more point of view before you make up your brilliant minds.
Dr. Cespedes, you seem to feel pretty strongly that hand-foot-mouth Hattie is the culprit.
Please share with us your reasoning based on the facts of the case.
I am taking the position that Handfoot Mouth Hattie is the one guilty of causing Kiara's rash.
Reason one.
Handfootmouth Hattie is also known to cause rashes, and specifically rashes on people's hands.
According to Kiara's fact frames and physical exam, she has a rash on the palms of both her hands and that's Kiara's main problem, the main reason she has been brought to our medical attention.
Reason number two.
Who can that be?
We're like in the middle of a debate.
I don't want to lose my train of thought.
And there it goes.
I completely understand, Dr. Cispedes.
Let me see who that is at the door.
We just wanted to know if you all wanted to break for a quick snack.
Yeah, pizza's all gone, but we raided your pantry and found some good stuff.
And since we're all stuck here... Well, what do you all think?
Dr Cespedes, maybe taking a really quick break could help you get your train of thought back on track, so to speak.
You know,
Sure.
Yeah.
Let's go fuel our brilliant brains.
Yes.
Y'all already know.
Love it.
Let's go wash our hands.
Yeah.
We'll be right back.
And now, a word from one of our sponsors.
Where the tutor fruits.
Is your plumbing system backed up?
Your tutor won't toot?
Your pooper won't poop.
You might have a crazy little thing called constipation.
Talk to your grown-up if you have belly pain, pain with pooping, or really smelly farts.
Smelly farts?
Gotta spread the love. find in the pantry.
I'm starved.
Hot potato chips.
I love that crunch.
I found apple slices in the fridge.
And my favorite, peanut butter and jelly beans.
I love PB&Js.
Pass the bread, please.
Pass the peanut butter, please.
The peanut butter is fantastic with the apple slices.
Is everything okay, Miss Immunity?
Peanut butter?
Peanut butter?!
Peanut butter?!
I despise peanut butter!
I don't even like peanuts!
I can't stand them!
I can't stand them!
Get the peanut butter out of here!
Miss Immunity!
What?!
!
What?
I don't like peanuts!
Peanut butter, peanut butter crackers, peanut butter chips and cookies or granola bars, peanut brittle, pad thai, things fried in peanut oil, peanut candies, peanut sauce.
Wait.
Why?
What's going on?
I despise peanuts!
Wait.
Misimmunity.
What on earth?
Never thought i'd see the day when miss superhero, miss perfect, miss immunity would totally lose her cool and over a peanut butter and jelly sandwich i could maybe see her getting upset, if she like.
Showed up to a party where someone had on the same outfit wait What?
Seriously, wait, whoa.
The peanut butter sandwich!
I wouldn't say that too loud if I were you.
In case you didn't notice, Miss Immunity's kind of on a rampage over here.
Are you thinking what I'm thinking?
I am.
Somebody move my aloe plant before she... Never mind.
Didn't Kiara have a peanut butter sandwich in the cafeteria?
It wasn't a peanut butter sandwich, but it had peanut butter in it.
And a bunch of other stuff.
Remember?
Anyway, Kiki, what did you bring for lunch?
My dad packed my lunch today, so I have a turkey sandwich, an extra pickles, carrot sticks and a juice.
How about you?
Oh, you know, my favorite sandwich ever.
I make it with grape jelly, banana, marshmallow fluff.
Extra peanut butter, raisins, and... Almost time to go.
We gotta hurry up.
Wait.
So Julissa, Kiara's friend, was the one who had the peanut butter sandwich.
Not Kiara.
But Elisa licked her fingers.
Okay, gross!
And she shared her markers with Kiara.
Hey, do you still need to borrow my markers to finish your masterpiece art project?
Yeah, do you have them?
Yeah, right here in my backpack.
Here.
Thanks, Julie.
So, even though she didn't eat the sandwich, there was probably traces of enough peanut butter that was still under lisa's hands, which was then transferred to kiara's hands and caused an allergic reaction to peanuts, and that's why miss immunity is losing her cool.
She really, really doesn't like peanuts.
She's the culprit.
Our initial educated guesses were incorrect.
It makes sense.
It all makes sense.
Now, the itchy lumpy red rash on her hands.
Hives.
Those lumps are called hives.
And the noisy whistling breathing?
Wheezing.
Wheezing can be part of an allergic reaction, too.
Or asthma, like my cousin has.
Correct.
But because the wheezing is happening right after being exposed to peanuts.
She's mainly wheezing because of that.
It's basically part of the allergic reaction that she's having.
But then, what about the cough and the rhinorrhea?
It's all down to timing.
The when in Kiara's fact frames.
She had the cough and rhinorrhea before she was exposed to the peanut residue on Julissa's hands.
Meaning that the cough and rhinorrhea were most likely part of an unrelated problem like an upper respiratory tract infection.
A common cold.
A common cold, right.
Kiara showed up to school that day with a simple common cold already.
I tried to keep it together.
I tried to keep quiet and let someone else take the blame.
Say what?
Shady boots.
You.
You incredibly dishonest, deceitful cell of the immune system.
Well, when I saw the peanut butter on Kiara's fingers, I had to spring into action and attack.
Attack, Kiara?
No, no.
Those petulant peanuts.
Even a trace, just a trace, makes me so upset that I can't even control myself.
I didn't mean to hurt Kiara.
Great work, many doctors, and especially you, many med students listening at home.
Because of your hard work, perseverance, and critical thinking, we can all say case closed.
Oh, you didn't hear what happened next.
Let me break it down for you.
Because we found out that kiara was having an allergic reaction to peanuts, miss spots knew she had to act, and fast.
To stop kiara's reaction, she gave her a very important medicine called epinephrine through an epinephrine auto injector what's that?
It kind of looks like a big fancy pen or marker, but it holds a tiny needle that can deliver the medicine to a person's body quickly in an emergency.
We're all relieved that Ms.
Spotts had one on hand in her office.
Do you know anyone who has a food allergy?
My sister is allergic to shellfish, crabs, and shrimp, so she carries one everywhere she goes.
Thanks for sharing that.
It is really important that she or a grown-up brings it wherever she goes.
You know places like a friend's birthday party.
She should also definitely have one at school with her as well.
And even though lots of people are understandably afraid to use it, it's actually pretty safe.
Once a person uses their auto injector the pen looking thing do need to go to the emergency room right away so the doctors and nurses there can watch them and make sure they're okay and that the reaction has truly stopped.
So what about Kiara?
She was back at school a couple of days later, I heard.
Oh, and you know what else I heard many medical students listening in?
Julissa still enjoys those very interesting sandwiches for lunch, but leaves the peanut butter out so that Kiara can safely eat lunch next to her, like they always have.
And...
I just can't get over Julissa calling Ms.
Spotts, the school nurse, when she saw something was wrong with Kiara.
It was a very good thing that she did that.
And you know something?
I hear that Kiara and Julissa are the very best of friends to this day.
Can we go now?
Sure, you can go.
Do you think you'll still make your movie, Mr. Measles?
Sorry about that.
There'll be reruns through the night.
Hey, one more for the road.
Knock, knock.
Who's there?
You.
Yoo-hoo.
Yoo-hoo.
Anybody want to share a comb with me?
No, seriously, everybody.
It's been real.
Real gross.
Bye, fun guy.
Miss Immunity, you look a lot more relaxed.
Ah, yes.
The epinephrine in Ms.
Spot's office and the steroids in the emergency room did the trick.
It's a good thing Chiara was able to get those things right away.
And no more peanuts!
Sounds like a good idea.
A really good idea.
That was a real doozy, huh, many doctors listening in?
Thanks for hanging in there.
You all did an amazing job.
I'm just dazzled by your detective skills.
Hey, who would have thought that Miss Immunity was the culprit behind Kiara's symptoms?
An allergic reaction.
